The following is a printable Member enrollment request form

1. Paste the form into your Word type program
Please pay close attention to letter case sensitive e-mail address.
Your e-mail address will be the only way I will contact you if there is a question.
2. Fill out the form in your Word type program
3. Print out the completed order form, affirm that you are an adult over 18 years of age by signing the form
4. Attach a Money Order in U.S. Funds Only Made out to: Sandra Lacey
5. Mail in to the address provided:
Eurochecks are accepted if made payable in U.S. Dollars
